Unbundling

What does Unbundling mean on a medical bill?

Unbundling is billing the parts of a procedure as separate lines when the payment rules treat them as one package. Each line looks small and reasonable on its own, and the total ends up above what the package would have paid. Sometimes it is legitimate: a genuinely separate service on the same day can be billed separately with the right modifier and the documentation to back it. Sometimes it is a billing habit nobody caught. Either way the records are what settle it.

What should I watch out for with Unbundling?

  • Several small lines on one date that each describe a piece of the same procedure
  • Ask whether those lines should have been billed under a single package code
  • A modifier added to split a package apart — ask what documentation supports it
