Denial code CO-11
What does Denial code CO-11 mean on a medical bill?
CO-11 means the reason recorded on the claim does not fit the service that was billed. Every claim carries a diagnosis code explaining why the care happened, and the plan checks whether it matches what was done. A mismatch usually comes from the code picked at the office rather than from anything that happened in the room. That is worth knowing, because a corrected claim fixes it once somebody compares the claim against the notes from that day. If the notes and the claim really do disagree, the correction is the whole job. Until it is fixed, the CO letters keep the amount on the provider's side of their contract.
What should I watch out for with Denial code CO-11?
- Ask the billing office which diagnosis code was submitted and have them read it back to you
- Ask your doctor's office to check that code against the visit notes and refile if it is wrong
- Ask your plan to reprocess once the corrected claim is in, instead of paying the balance first
